Job summary

• Licensed in RN, Social Worker, respiratory, physical, occupational, or speech-language pathology.
• Case management/care coordination experience in inpatient settings preferred.
• English proficiency; MS Office proficiency.

Responsibilities

• Coordinate the interdisciplinary care plan and discharge planning.
• Document patient care activities per CMS IPoC guidelines.
• Schedule family conferences and communicate progress.
• Lead weekly interdisciplinary team conferences.
• Identify barriers to discharge and develop solutions.
• Collaborate with HIM for CMG reviews.
