Job summary

Virginia Mason Rehabilitation, part of Lifepoint Health, is seeking a Case Manager to support patient care coordination and discharge planning within a dynamic multidisciplinary team.

You will complete orientation, collaborate with staff, document IPoC per CMS, schedule family conferences, lead weekly care team meetings, review CMG, and handle additional duties as assigned. We offer comprehensive benefits, tuition assistance, PTO, and career growth opportunities.

Qualifications

• Bachelor's degree from an accredited nursing school or social work program.
• Basic Life Support (BLS) certification required within 30 days of hire.
• ACLS and PALS certifications required within six months of hire.
• One year of experience preferred.
• ASLS certification preferred.

Responsibilities

• Complete departmental orientation as well as initial and annual competencies.
• Collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge.
• Complete documentation in accordance with workflow timelines and content standards, including the Individual Plan of Care (IPoC) per CMS guidelines.
• Schedule and conduct family conferences and caregiver communications following team meetings or as needed.
• Facilitate weekly care team conferences to review patient progress and adjust treatment plans.
• Review the assigned Case Mix Group (CMG) and assist the care team in identifying and documenting treated comorbid conditions; notify the Health Information Management (HIM) team as needed.
• Perform other duties as assigned.
