Job summary

Qualifications

• A minimum of two years of satisfactory employment experience working in direct case management, social services or related field.
• Or any equivalent combination of training and experience which provides the required knowledge, skills, and abilities.

Responsibilities

• Manages and monitors assigned caseload of Beneficiaries participating in a rehabilitation program; develops program plan/goals and evaluates beneficiaries progress by conducting routine meetings with Beneficiaries and/or staff.
• Serves as advocate for Beneficiaries in order to acquire services that will enable them to functionally cope with their environment.
