PAM Health Rehabilitation Hospital of Tulsa is seeking a Clinical Navigator to be the face of our hospital, building trusted relationships with patients, families, physicians, and referral sources. In this role, you will cultivate referral relationships, manage assigned territory, and coordinate patient assessments to support admissions.

Qualificações

• Degree in a health-related field from an accredited college or university.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy.
• Minimum two years’ experience in the designated field of license, preferably with rehabilitation/LTACH and marketing experience.

Responsabilidades

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and conducts sales contacts.
• Collaborates with Admissions Manager, nursing staff, and payer representatives to facilitate referrals.
• Provides feedback for program development and quality improvement initiatives related to customer service and admissions.
• Maintains excellent relationships with prospective patients, families, physicians, discharge planners, case managers, and payers.
