Complete Care in Melbourne, FL, seeks a Care Coordinator (Rehab) to support patient recovery by delivering provider-directed therapies, preparing patients for sessions, and guiding them through treatment plans. This role emphasizes organization, compassion, and accurate documentation.

You will use the EHR to record therapies, educate patients on exercises and DME, coordinate with clinicians, and maintain a clean treatment area while supporting daily clinic operations.

Qualifications

• Assist in delivering rehab therapies safely and accurately.
• Prepare patients for therapy and explain what to expect.
• Help patients stay on track with prescribed treatment plans.
• Document therapies accurately in the EHR (Electronic Medical Record).
• Educate patients on therapy exercises and proper use of DME.
• Monitor patient progress and communicate concerns to providers when appropriate.
• Coordinate with providers and teammates to ensure seamless patient care.
• Maintain an organized treatment area while supporting daily clinic operations.
• Deliver every patient interaction with professionalism, compassion, and care.
