Qualifications

• Bachelor’s Degree in Human Services or related clinical field.
• Two years of case management experience preferred.
• Care Coordinator II requires ACM or CCM certification.

Responsibilities

• Regular and predictable attendance is essential.
• Provide coordinated, goal-based rehabilitation from intake to discharge to maximize outcomes and satisfaction.
• Lead the treatment team in client-focused problem solving and coordinate information for external parties.
