Job summary

InnovAge Holding is hiring a Rehabilitation Aide to assist with participant treatments and department maintenance under supervision of therapists. You will support home visits, equipment handling, and treatment preparation within a patient-centered care model.

The role emphasizes preserving independence for seniors, with responsibilities split between participant care and administrative tasks. A valid driver’s license and experience with the frail or elderly are preferred.

Qualifications

• High School diploma or GED.
• Requires personal transportation, state issued driver’s license, good driving record and auto insurance as required by law.
• A minimum of one year experience working with the frail or elderly.

Responsibilities

• Transports and prepares participants for treatment and provides support services.
• Applies hot/cold packs and paraffin as assigned.
• Monitors vital signs and reports abnormalities to the supervising therapist.
• Assists with ambulation, transfers and mobility per POC.
• Delivers and retrieves DME from clinics and participant homes.
