AltaMed Health Services Corp. is seeking a Rehabilitation Aide to perform or assist with personal care ADLs/IADLs for participants at home or at the PACE Center.

Under the guidance of a licensed Therapist, you will assist with treatments for physical, mental, and occupational disabilities, following a treatment plan to improve patients' functional abilities in a geriatric population. Qualifications

• High school diploma or equivalent required.
• Restorative Nurse Assistant Certification preferred.
• 1–2 years experience with frail elderly and disabled populations preferred.
• Bilingual English/Spanish/Cantonese/Mandarin preferred.

Responsibilities

• Perform or assist with personal care ADLs/IADLs for participants at home or PACE center.
• Assist with treatments for physical, mental and occupational disabilities under supervision of a licensed Therapist.
• Follow through with a treatment plan to increase, restore and maintain functional capabilities.
• Focus on providing exceptional customer experience and quality care to geriatric population.
