Job summary

Qualifications

• Administers all types of medications correctly per age group for all populations served according to policy.
• Demonstrates proficient technical skills and operational knowledge of all equipment used on the unit.
• Performs daily rotational task assignment as directed.
• Supervises Nursing Assistants in basic nursing functions and ensures safe ambulation, transfers, and comfort measures.
• Serves as a preceptor as necessary.
• Communication/Collaboration.
• Data collection/assessment of all populations served and primary chart assessments.
• Documents/communicates patient data clearly; executes physician orders; HIPAA compliance.
• Collaborates on Care Plan input.
• Timely collaboration to establish priorities in planning and implementing patient care.

Responsibilities

• Conduct comprehensive patient assessments through observation, interviews, and physical exams to gather history and vital signs.
• Develop and implement individualized care plans with SMART goals in collaboration with patients and teams.
• Administer medications and treatments safely and perform procedures like wound care and IV therapy.
• Educate patients and families about health conditions and self-care; provide discharge instructions.
• Advocate for patient safety and confidentiality; secure resources to improve outcomes.
• Engage in quality improvement and evidence-based practice to enhance care.
• Collaborate with doctors, therapists, social workers, and other providers for coordinated care.
• Collect and analyze patient health data; prioritize needs based on condition.
• Document care activities in medical records for continuity.
• Evaluate and adjust care plans based on ongoing assessments.
• Perform nursing procedures such as wound care and catheterization.
• Ensure patient safety by following policies and participating in bedside shift reports.
• Performs other duties as assigned.
