Job summary

Qualifications

Responsibilities

• Conducts comprehensive assessments and notifies physicians of significant changes.
• Develops, directs and evaluates individualized care plans with patient/family education and discharge planning.
• Prepares patients for procedures and administers physician orders with safety protocols.
• Regularly reassesses patient responses, adapts care to changing needs and communicates modifications to physicians.
• Provides thorough patient updates and efficient handoffs using bedside reporting.
• Contributes to safety, quality initiatives and patient satisfaction efforts.
