Job summary

Qualifications

Responsibilities

• Patient assessment & care: conduct comprehensive assessments and notify physicians of changes.
• Care planning & coordination: develop, implement, and evaluate individualized care plans with education and discharge planning.
• Medication administration and safety: prepare patients for procedures, ensure orders are followed and safety protocols are in place.
• Ongoing evaluation: reassess responses, adjust care as needed, communicate with physicians for modifications.
• Handoffs & communication: provide thorough updates and ensure smooth transitions of care.
• Safety & quality: uphold safe environment and participate in quality initiatives.
