Job summary

Penn State Health Inpatient Rehabilitation Hospital is seeking a Clinical Liaison (RN, OT, PT, or SLP) in Hummelstown, PA. The role blends clinical expertise with relationship-building to coordinate post-acute care and admissions from referring facilities.

You will advocate for referred patients during pre-admission, educate healthcare staff, and collaborate with physicians and families to develop care plans, while facilitating smooth patient transfers and addressing barriers to access.

Qualifications

• Current and valid state licensure for RN/OT/PT/SLP.
• Multi-state licensure required based on geography.
• Two years clinical healthcare experience preferred.

Responsibilities

• Develops relationships with hospitals and referral sources; educates stakeholders on rehabilitation services.
• Educates staff, patients and families about rehabilitation services.
• Manages referral processes and admission coordination from referring hospitals.
• Supports Sales and Marketing through programs, tours and meetings in assigned hospitals.
• Identifies opportunities to grow market share and coordinates with leadership.
