Banner Rehabilitation Hospital - Phoenix Downtown is seeking a Clinical Liaison (RN, OT, PT, or SLP) to advocate for referred patients and coordinate care plans with physicians and families, supporting pre-admission activities and transfers.

This role builds relationships with case managers and hospitals, educates referral sources about rehabilitation services, and helps expand program access within a growing post-acute network.

Qualifications

• Current and valid state licensure as RN, OT, PT, or SLP; multi-state licensure based on territory.

Responsibilities

• Lead relationships with case managers, physicians and clinicians in hospitals; educate staff and families about rehabilitation services.
• Assess rehab patients at referring hospitals for admission and manage the referral process.
• Coordinate care plans with physicians and families; facilitate transfers to post-acute care.
• Support sales/marketing through educational programs, tours and meetings in assigned hospitals.
• Identify opportunities to expand strategic business initiatives with leadership.
