The Phoenix Post Acute in Texas City, TX is seeking a Director of Rehabilitation/Therapy Program Manager. You will lead the in-house rehab team, create and implement rehabilitation programs, and oversee daily operations with a focus on clinical excellence and inter-departmental collaboration.

Ideal candidates are licensed PT/OT/SLP and PDPM aware, with 3+ years of leadership in long-term care rehab. This role offers ownership of the program and an attractive leadership incentive plan.

Qualifications

• Licensed PT/OT/SLP with active state license.
• 3+ years clinical experience in rehabilitation.
• Supervisory/administrative experience in long-term care rehab preferred.
• Knowledge of Medicare PDPM and payor management.

Responsibilities

• Implements and monitors rehab policy and procedures.
• Supervises rehab staff and coordinates orientation and competency.
• Coordinates admissions and caseload distribution.
• Manages Medicare PDPM case tracking and billing metrics.
• Recruits rehab staff and maintains regulatory compliance.
• Maintains relationships with hospital therapists and physicians.
