Job summary

The Phoenix Post Acute in Texas City, TX seeks a Director of Rehabilitation/Therapy Program Manager to lead our in-house therapy team and shape clinical direction.

You will create and implement new rehabilitation programs, supervise staff, coordinate admissions, and manage daily operations with strong leadership support, ensuring patient-centered care and compliance with PDPM standards across the facility.

Qualifications

• Licensed Physical, Occupational, or Speech Therapist.
• At least 3 years of therapy experience.
• Supervisory/administrative experience in long-term care preferred.
• Knowledge of Medicare PDPM and payor management.

Responsibilities

• Lead a strong therapy team and collaborate with inter-departmental teams.
• Develop and implement rehabilitation programs tailored to the facility community.
• Continue to treat patients while guiding the therapy staff.
• Manage therapy department operations and ensure regulatory compliance.
• Monitor metrics, reimbursement processes, and facility performance.
