Responsibilities

• Implement treatment plans and select appropriate activities to achieve patient goals, revising plans with patient and family input.
• Provide ongoing education, discharge instructions, follow-up, and referrals to community resources.
• Collaborate with physicians to establish and evaluate treatment programs, goals, and discharge plans.
• Document evaluations, treatment sessions, and patient education according to departmental guidelines.
• Maintain ethical standards, confidentiality, punctuality, and professionalism, including effective communication and teamwork.
