Mercy Health is seeking a Patient Care Associate for the Rehab unit at Lorain Hospital. Under RN guidance, you will provide essential patient care, monitor vitals, assist with mobility, and ensure accurate documentation in EPIC.

Excellent communication and a patient-centered approach are required. The role supports a 36-hour work week, 7:00 AM to 7:30 PM shifts, with every third weekend and alternating weekends.

Qualifications

• Experience in acute care or similar clinical settings preferred.
• Familiarity with patient care tasks and documentation.
• Ability to follow policies and communicate with the healthcare team.

Responsabilités

• Provide basic patient care under RN guidance, including vital signs, hygiene, and mobility.
• Serve as liaison between patient and nurse; report changes promptly.
• Deliver high level customer service to patients, families, and staff.
• Document patient care accurately and timely in the EHR.
