Job summary

PAM Health Rehabilitation Hospital of Katy in Houston is seeking a Clinical Navigator to lead referral development and patient assessments. You will build relationships with physicians, discharge planners, and payer representatives to optimize admissions and care transitions.

The role collaborates with the Business Development team under the Director of Business Development to grow referrals and improve patient experiences in our LTACH setting.

Qualifications

• Education and Training: Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships, manages assigned territory, and completes patient assessments.
• Makes sales contacts and analyzes referral/admission data from the territory.
• Collaborates with Admissions Manager, admission staff, nursing, and internal/external staff to facilitate referral conversion.
• Develops and maintains relationships with stakeholders including prospective patients, families, physicians, discharge planners, case managers, and payer representatives.
