Job summary

Mountain-View-Healthcare-Center is seeking a licensed nurse (LVN/RN) to coordinate rehabilitation services and advocate for patients. The role involves admissions, care coordination, and payer communications within a hospital-like setting.

Responsibilities include documenting payer interactions, coordinating with the interdisciplinary team, and ensuring accurate census and payments for managed care and Medicare residents. Full-time position with comprehensive benefits.

Qualifications

• Current California LVN or RN license in good standing.
• Minimum 3 years experience in a hospital, nursing, or other related health care facility.
• Working knowledge of managed care rules, regulations, and guidelines.
• Outstanding communication (oral and written) skills.

Responsibilities

• Greet new patients/residents ensuing a positive admission experience.
• Evaluate and communicate resident status, changes in functions and care plans to payers.
• Document all payer interactions regarding resident progress, expected outcomes and special instructions.
• Meet with interdisciplinary team to coordinate services ensuring resident's total regimen of care is maintained.
• Coordinate and communicate with business office and medical records to ensure accurate census and payment of managed care and Medicare residents.
