• Follow up with residents’ post-discharge outcomes.
• Communicate resident status and care plan changes to payer by phone or in writing.
• Document payer interactions and outcomes.
• Coordinate cost-effective care with facility staff and disciplines.
• Meet with the interdisciplinary team to ensure care plan alignment and service coordination.
