Job summary

Qualifications

• Bachelor's degree is required and two years of case management experience with disabled populations or rehabilitation setting is preferred.
• Knowledge of counseling techniques and conflict resolution, and ability to document per facility standards.
• Excellent communication skills and ability to work as part of a treatment team.

Responsibilities

• Reviews pre-admission documentation in a timely manner.
• Conducts family and patient interviews during intake.
• Files IPP-related documents in patient records and writes weekly contact notes.
• Leads IPP meetings, family conferences, discharge IPPs and special meetings as chairperson.
• Provides advocacy and ongoing patient and family education on progress and plans.
• Coordinates scheduling for SSDI, Medicare, home evaluations and outside evaluations.
• Assists with discharge planning and post-discharge referrals with Follow-up Specialist.
