Job summary

Rehab Without Walls Neuro Rehabilitation in Austin, TX is seeking a Rehabilitation Case Manager (PT/OT/SLP/LCSW) to lead a transdisciplinary neuro-rehabilitation program. You will coordinate patient care, develop treatment plans, and monitor outcomes with clinicians, families, and payers.

Qualifications

• Current Therapeutic License in state of practice (OT, PT, SLP, LCSW, LPC, NP, Rehabilitation Counseling/Psychology).
• Maintains relevant professional licensure/registration/certification in the state of practice.
• CBIS certification (preferred).
• BLS First Aid and CPR certifications.
• Minimum of two years’ experience in a rehabilitation setting with neurologically impaired patients.
• Prior case management and/or insurance administration experience (preferred).
• Demonstrates excellent organization, problem solving, decision making and communication skills.
• Organized and able to function under minimal supervision.
• Has reliable transportation and ability to travel between multiple assignments/locations daily.

Responsibilities

• Manages provision of individual patient services and coordinates rehabilitation plans with clinical staff.
• Formulates transdisciplinary rehabilitation treatment plans and clinical outcomes with patient and team.
• Completes CC evaluation and develops initial, progress and discharge reports; monitors status.
• Monitors satisfaction of patient, family, physician and payers with the program.
• Leads interdisciplinary case conferences and communicates Treatment Plans/Progress Reports to physicians and family.
• Schedules and facilitates conferences with clinical team, payers, physicians and family.
