Job summary

Responsibilities

  • Reviews pre‑admission documentation in a timely manner.
  • Conducts family and patient interviews during intake.

  • Attends treatment meetings for caseload patients.
  • Serves as chairperson for IPP meetings and family conferences.
  • Advocates for the patient and educates family on progress.

  • Provides IPP copies to patient or authorized persons.
  • Explains long‑term IPP goals with licensed therapist.
  • Assists with scheduling SSDI, Medicare, and Home Evaluations.
  • Coordinates outside evaluations with treatment team.
  • Arranges outside activities with program staff (e.g., classes).
  • Explains financial needs for activities and medications and coordinates funds.
  • Explains rules to family and patient rights; seeks outside referrals as needed.
  • Arranges travel for weekends/holidays and emergencies.
  • Develops discharge plans prior to graduation; coordinates with Follow‑up Specialist.
  • Available after hours as needed and adheres to policy and deadlines.
  • Performs other duties as assigned.

Job description

General Statement of Duties

The Case Manager will be responsible for providing all case management services under the supervision of the Program Director. The Case Manager shall attend all IPP’s and weekly rounds to gather information, provide input to other therapy members, and facilitate the team process to help patients maximize their rehabilitation potential and achieve successful discharge from the program. The Case Manager will facilitate all conferences with family and/or referral sources, assist with educating them on the patient’s progress, and promote the patient’s participation in the program by performing advocacy services. Regular contact with the patient’s family and monthly or as-needed contact with the funding source are required. The Case Manager must demonstrate flexibility within the interdisciplinary structure, uphold the mission, philosophy, and goals of the organization, maintain confidentiality of personnel and patient data, and ensure all duties are performed with professionalism and in accordance with organizational policy and accreditation standards.

Essential Functions
  • Reviews the patient's pre‑admission documentation in a timely manner.
  • Conducts family and patient interviews during the intake process.
  • Ensures IPP’s, case‑management notes, and other relevant documentation are filed in the patient’s medical record.
  • Writes weekly contact notes and files them in the medical record.
  • Attends all treatment meetings concerning patients on the caseload.
  • Serves as chairperson for IPP’s, family conferences, discharge IPP’s, and special meetings/conferences.
  • Performs advocacy services for the patient.
  • Discusses plans, goals, and status with the patient and significant others to ensure involvement and education.
  • Provides a copy of the Individual Program Plan (IPP) to the patient or by request of significant others after proper releases are obtained.
  • Explains long‑term goals of the IPP in conjunction with the licensed therapist.
  • Assists with scheduling appointments such as SSDI, Medicare, and Home Evaluations.
  • Assists the patient with scheduling outside evaluations as deemed appropriate by the treatment team.
  • Arranges outside pursuits in coordination with program staff, such as classes at local colleges and church.
  • Explains the amount of money needed to cover planned activities, medication needs, or other needs and may coordinate receipt of these funds when needed.
  • Explains rules and regulations to the family and the rights of the individual being served.
  • Seeks advice from outside referral sources to intervene in special problems that may arise.
  • Assists the individual and family regarding travel arrangements for weekends, holidays, and emergencies.
  • Coordinates and assists with development of discharge plans before graduation.
  • Provides referral information in coordination with the Follow‑up Specialist to discharge the community.
  • Available after hours as needed.
  • Performs duties in accordance with Moody Neuro organizational policy, CARF, and other accrediting bodies with full completion of documentation within expected timeframe.
  • Performs other duties as assigned.

Minimum Qualifications
  • Knowledge of counseling techniques and conflict resolution practices and the ability to implement them therapeutically.

  • Knowledge of the multiple and complex needs of the brain‑injured population.
  • Knowledge of and ability to document according to facility standards.
  • Excellent communication skills.
  • Ability to work as a team member and facilitate the efforts of the treatment team.
  • Ability to participate in the successful implementation of overall program goals.
  • Ability to cope with stressful situations.
  • Knowledge of licensing and accreditation standards (e.g., CARF, TRC, Medicare/Medicaid).
Lifting Requirements

Individuals in clinical care positions are required to lift items independently up to 20pounds.

Education

Bachelor’s Degree in rehabilitation, psychology, social science, or an accepted field from an accredited college or university. Master’s degree preferred.

Experience

Two (2) years’ experience in Case Management Services with one or more disabled populations, or two years’ experience working with persons with disabilities in a rehabilitation setting.
