Encompass Health is seeking a Case Manager in a Miami, FL area role designed to connect patients with comprehensive rehabilitation care. You will lead interdisciplinary planning from admission through discharge, coordinate resources, and support families to ensure smooth transitions at every step.

The position requires an active clinical license and CCM/ACM certification or plan to obtain within two years. A bachelor's degree is required, with nursing candidates often preferred.

Qualifications

• Must possess a professional license within the state of practice or an eligible clinical credential.
• CCM or ACM certification required or to be obtained within two years of hire.
• At least a bachelor’s degree; graduate degree preferred for non-nursing roles.

Responsibilities

• Coordinate patient care from admission to post-discharge with an interdisciplinary team.
• Develop and refine tentative discharge plans and contingency strategies.
• Educate patients and families on rehabilitation goals and the case manager role.
• Monitor patient experience and ensure timely, appropriate care across payors.
• Attend and contribute to team conferences and care plan modifications.
• Maintain documentation and compliance with regulations and policies.
• Identify trends from case management data and coordinate post-discharge referrals.
