Job summary

• Counseling skills and knowledge of community resources.

Responsibilities

• Document discharge issues and insurance-related progress in chart.
• Coordinate with interdisciplinary team and families for smooth discharge.
• Communicate with insurance providers for authorizations.
• Ensure compliance with regulatory standards and CARF requirements.
