Job summary

Encompass Health is seeking a Case Manager to bridge exceptional care and patient outcomes in Athens, GA. You will coordinate patient care from admission through post-discharge, guiding interdisciplinary plans and ensuring timely, quality discharge processes.

The role emphasizes clinical expertise, regulatory awareness, and proactive communication with patients and families to support meaningful rehabilitation journeys.

Qualifications

• Must be able to independently complete an assessment within scope of practice.
• Active license in the relevant discipline.
• CCM or ACM certification must be obtained within two years of starting.

Responsibilities

• Work with interdisciplinary team to guide treatment plans based on patient needs.
• Coordinate tentative discharge plans and contingency plans with the team.
• Participate in planning and execution of patient discharge experience.
• Monitor patient experience including quality, timeliness, and payor satisfaction.
• Attend and contribute to weekly team conferences and update treatment plans as needed.
• Complete case management documentation and addendums promptly.
