Job summary

Wellstar Health System seeks a Social Worker/Case Manager for inpatient rehab to coordinate discharge planning, provide education to patients and families, and ensure alignment with NASW standards. You will liaise with the rehab team, insurers, and outside agencies to facilitate timely, compliant care plans.

Required are 2+ years in medical case management, Licensure (CCM/LCSW/LMSW/MSW or RN) and strong computer and counseling skills.

Qualifications

• Associates Nursing or Bachelors in Nursing or MSW required.
• CCM / LCSW / LMSW / MSW or RN license required at hire.
• Minimum 2 years in medical case management inpatient rehab.

Responsibilities

• Review new admissions in EPIC and paper charts for compliance.
• Document discharge issues and coordinate care until discharge.
• Communicate with insurance for authorization and coverage.
• Enter precert information into CANOPY and coordinate financials.
• Coordinate interdisciplinary team meetings and family education.
