Job summary

Wellstar Health System seeks a Care Coordination Social Worker to assess psychosocial needs and coordinate discharge planning across the continuum. You will partner with patients, families, and the care team to ensure timely, safe transitions to the next level of care.

Responsibilities include psychosocial assessments, referrals to community resources, and collaboration with RN Case Managers to address social determinants of health. GA-based hospital system with a mission-driven culture.

Qualifications

• Minimum education: Bachelor’s or Master’s in Social Work.
• Licensing: BLS required; GA preferred licenses.
• Experience: healthcare setting preferred.

Responsibilities

• Perform psychosocial assessments and plan discharge.
• Coordinate with care teams for safe transitions.
• Identify barriers to timely disposition and address them.
• Provide resources and referrals for community services.
• Document assessments and actions in the medical record.
• Engage in interdisciplinary rounds and care planning.
