Centra Rehabilitation Hospital, part of Lifepoint Health, seeks a PRN Non-Licensed Case Manager to coordinate care and discharge planning for patients in an inpatient setting.

The role requires a social work degree (BSW/MSW), CCM/ACM certification preferred, and strong English communication skills. Experience in inpatient settings is highly preferred; familiarity with CMS IPoC guidelines is a plus.

Qualifications

• Bachelor's or Master's degree in social work, or as required by state regulations.
• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
• Minimum of 2 years of social work or case management experience in an inpatient setting preferred.

Responsabilités

• Complete departmental orientation and annual competencies.
• Collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge.
• Document according to workflow timelines and CMS IPoC guidelines.
• Schedule and conduct family conferences and caregiver communications.
• Facilitate weekly care team conferences to review progress and adjust treatment plans.
