Job summary

Centra Rehabilitation Hospital, part of Lifepoint Health, seeks a PRN Case Manager in Rehabilitation to coordinate care, plan discharges, and communicate with caregivers and the healthcare team in Lynchburg, VA. The role supports development and execution of individualized care plans meeting CMS guidelines.

You will work with a multidisciplinary team to identify barriers to discharge, document per standards, and lead family conferences while advancing patient progress and treatment plans.

Qualifications

• Bachelor's or master's degree in social work, or as required by state regulations.
• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
• Minimum of 2 years of social work or case management experience in an inpatient setting highly preferred; experience in acute or rehabilitation hospital preferred.
• Strong verbal and written communication skills in English; additional languages preferred.
• Proficiency in Microsoft Office applications.

Responsibilities

• Complete departmental orientation as well as initial and annual competencies.
• Collaborate with interdisciplinary staff to identify and resolve barriers to care or discharge.
• Complete documentation in accordance with workflow timelines and content standards, including the IPoC per CMS guidelines.
• Schedule and conduct family conferences and caregiver communications following team meetings or as needed.
• Facilitate weekly care team conferences to review patient progress and adjust treatment plans.
