Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our team in a SNF setting. You will evaluate and treat patients, develop plans of care, and work with facility staff to ensure quality outcomes.

You will document progress and re-evaluate plans as patients progress, while supervisin g Occupational Therapy Assistants. Strong communication with patients and families is essential to explain progress, expected outcomes and potential changes in treatment.

Qualifications

• Must hold an active OT license in good standing.
• Experience evaluating and treating patients in SNF settings preferred.
• Ability to create measurable functional goals and plans of care.

Responsabilités

• Evaluate and treat patients in SNF following individualized care plans.
• Develop plans of care with measurable functional goals.
• Re-evaluate patient progress and revise plans as needed.
• Complete timely documentation including evaluations, re-evaluations, discharges and daily progress notes.
• Supervise Occupational Therapy Assistants and collaborate with facility staff.
