Job summary

Enhance Rehabilitation is seeking Occupational Therapist candidates to join our team in a SNF setting. You will evaluate and treat patients, develop plans of care, and set measurable goals, while collaborating with facility staff to ensure high-quality care.

You will re-evaluate progress, adjust care plans as needed, and maintain thorough documentation. Supervision of OTAs is also a key responsibility in this role.

Qualifications

• OT licensure and eligibility to practice (as applicable).
• Ability to develop individualized plans of care and measurable goals.
• Strong communication with patients, families, and staff.

Responsibilities

• Evaluate and treat patients in SNF following individualized care plans.
• Develop plans of care and set measurable, functional goals.
• Treat patients, answer concerns, and educate on injury, progress and outcomes.
• Re-evaluate patient progress and revise plans of care accordingly.
• Complete timely documentation including evaluations, progress notes and discharges.
• Supervise Occupational Therapy Assistants and collaborate with facility staff.
