Job summary

Enhance Rehabilitation seeks qualified Occupational Therapist candidates to join our growing team in a SNF setting. You will evaluate and treat patients following individualized plans of care, while collaborating with facility staff and educating patients and families on progress and outcomes.

Responsibilities include developing measurable goals, providing excellent patient service, supervising therapists assistants, and maintaining timely documentation to support evaluations and discharges.

Qualifications

• Evaluating new patients, developing plans of care and setting measurable, functional goals.
• Supervising Occupational Therapy Assistants and ensuring quality patient care.
• Completing timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.

Responsibilities

• Evaluate and treat patients in a SNF setting following an individualized plan of care.
• Collaborate with facility staff to ensure excellent patient care.
• Re-evaluate progress, revise plans of care and adjust goals as needed.
• Provide patient and family education on injury, progress and outcomes.
