Job summary

Enhance Rehabilitation is seeking an Occupational Therapist to join our team in a SNF setting. You will evaluate and treat patients, develop individualized plans of care, and collaborate with facility staff to ensure optimal outcomes.

You will re-evaluate progress, adjust goals, educate patients and families, and maintain timely documentation. Supervising Occupational Therapy Assistants is also part of the role.

Qualifications

• Evaluate patients to determine functional needs and goals.
• Develop and monitor individualized plans of care.
• Document evaluations, progress notes, and discharges in a timely manner.

Responsibilities

• Evaluate and treat patients in SNF following an individualized plan of care.
• Collaborate with facility staff to ensure excellent patient care.
• Educate patients and families on injury, progress and outcomes.
• Re-evaluate progress and revise plans of care as needed.
• Complete timely and accurate documentation including evaluations and progress notes.
• Supervise Occupational Therapy Assistants.
