Job summary

Enhance Rehabilitation is seeking qualified Occupational Therapist candidates to join our team. Qualifications

• OT degree and state licensure required
• Experience evaluating and treating patients in SNF setting preferred
• Ability to develop goals and plans of care and document progress

Responsibilities

• Evaluate and treat patients in SNF setting following individualized care plans
• Develop plans of care and measurable functional goals
• Collaborate with facility staff to ensure high-quality care
• Complete timely documentation: evaluations, re-evaluations, discharge notes and daily progress notes
• Supervise Occupational Therapy Assistants
• Educate patients and families about injuries, progress, and outcomes
• Re-evaluate progress and revise plans of care as needed
