Job summary

Harborview-Health-Systems seeks a dedicated Registered Nurse in Tarboro, NC to supervise CNAs and coordinate care within a skilled nursing setting. You will work with the IDT to assess needs, plan care, and ensure timely assessments and documentation.

Qualifications

• Active RN license in good standing in the state of practice.
• Experience in skilled nursing or long-term care preferred.

Responsibilities

• Supervise and support CNAs under the direction of the Director of Nursing
• Collaborate with the interdisciplinary team (IDT) to assess, plan, implement, and evaluate individualized care plans
• Ensure initial baseline and periodic comprehensive assessments are completed on time
• Prepare for and facilitate physician rounds by flagging concerns and preparing documentation for review
• Initiate consultations or referrals as appropriate based on resident needs
• Perform administrative tasks including medical documentation, evaluations, and reports
• Deliver nursing care within scope of practice and aligned with facility policy and clinical standards
• Monitor residents for changes in condition (e.g., confusion, fever, breathing difficulty, pain, appetite changes, mobility loss) and notify providers as needed
• Perform other duties as assigned by the Director of Nursing
