Job summary

Gulf Valor Rehabilitation in Pensacola, FL is seeking a dedicated Registered Nurse to join our faith-based facility and support residents through compassionate, skilled care. You will supervise CNAs, collaborate with the IDT to assess and implement individualized care plans, and ensure timely assessments and physician rounds.

This role emphasizes documentation, care within policy, and ongoing professional development; background in long-term care is valued, and a strong nursing presence helps

Qualifications

• Active RN license in good standing in the state of practice.
• Experience in a skilled nursing or long-term care setting is preferred but not required.
• Ability to supervise CNAs and coordinate with the IDT.
• Strong documentation and communication skills.

Responsibilities

• Supervise and support CNAs under the direction of the Director of Nursing.
• Collaborate with the interdisciplinary team to assess, plan, implement, and evaluate individualized care plans.
• Ensure initial baseline and periodic assessments are completed on time.
• Prepare for and facilitate physician rounds by flagging concerns and preparing documentation for review.
• Initiate consultations or referrals as appropriate based on resident needs.
• Perform administrative tasks including medical documentation, evaluations, and reports.
• Deliver nursing care within scope of practice and aligned with facility policy and clinical standards.
• Monitor residents for changes in condition and notify providers as needed.
• Perform other duties as assigned by the Director of Nursing.

We are currently seeking a dedicated Registered Nurse

What You Will Do

• Supervise and support Certified Nursing Assistants (CNAs) under the direction of the Director of Nursing
• Collaborate with the interdisciplinary team (IDT) to assess, plan, implement, and evaluate individualized care plans
• Ensure initial baseline and periodic comprehensive assessments are completed on time
• Prepare for and facilitate physician rounds by flagging concerns and preparing documentation for review
• Initiate consultations or referrals as appropriate based on resident needs
• Perform administrative tasks including medical documentation, evaluations, and reports
• Deliver nursing care within scope of practice and aligned with facility policy and clinical standards
• Monitor residents for changes in condition (e.g., confusion, fever, breathing difficulty, pain, appetite changes, mobility loss) and notify providers as needed
• Perform other duties as assigned by the Director of Nursing

What You Must Have

• Active RN license in good standing in the state of practice.
