Job summary

Saint Francis Hospital, a Trinity Health Mid-Atlantic member, is seeking an experienced Registered Nurse to join the wound care team. Schedule: Part-time, Monday, Wednesday, and Friday, 8am-4pm; duties include comprehensive wound assessments, documentation, care planning, and patient/family education to promote healing.

The role collaborates with physicians, physical therapists, and other nurses to implement clinical pathways, manage in-patient and out-patient wounds, and maintain skin

Qualifications

• Current licensure as a Registered Nurse in the State of Delaware required.
• ASN required, BSN preferred, from an accredited school of nursing.
• At least 3 years' of clinical experience required, preferably in Wound Care.
• Wound Care certification preferred; eligible to obtain within 6 months of hire.

Responsibilities

• Adheres to hospital nursing practice standards and plans patient care.
• Implements nursing actions for each patient identified in wound care.
• Detailed history and physicals for every patient in the wound center.
• Discusses care plans with patient/family and documents outcomes.
• Collaborates with multidisciplinary team to deliver patient care.
• Evaluates patient understanding and provides written instructions.
• Maintains charts and timely documentation; reports patient status as needed.
• Initiates Clinical Pathways/Plan of Care on first visit and updates as needed.

Employment Type:Part timeShift:Day ShiftDescription:Saint Francis Hospital, a member of Trinity Health Mid-Atlantic, is seeking an experienced Registered Nurse to join the team!

Schedule: Part-time, Monday, Wednesday, and Friday - 8am-4pm

The responsibilities of the wound care nurse involve conducting and documenting detailed physical exams, evaluating and managing patients with wounds and extensive case management. Some of the case management functions include but are not limited to assessing and providing for wound care needs post discharge, collaborating with providers and other medical personnel and ordering equipment/supplies. This position involves working with the wound care team including physicians, physical therapists and other nurses. The nurse will be involved with educating patients and families about chronic wounds and maintaining appropriate follow up care. This nurse will be responsible for managing in-patients as well as out-patients with complex wounds, treating patients through clinical pathways, following standards of care and advanced clinical practice. This position is responsible for educating and supporting in-patient nurses on maintenance of skin integrity, prevention and treatment of skin breakdown and wounds.

Responsibilities include, but not limited to:

• Adheres to hospital nursing practice standards, demonstrates appropriate history and physical assessment, planning, implementation and evaluation of patient care.

• Implements appropriate Nursing actions to each patient that is identified.

• Thorough History and Physical’s will be done on every patient in the wound center.

• Makes decisions that reflect both knowledge of facts and sound judgment.

• Discusses with and includes patient and/or family significant other about planned patient care and expected outcomes and documents it.

• Identifies and appropriately utilizes multidisciplinary approach to facilitate delivery of patient care.

• Evaluates the patient’s/significant other’s understanding of instructions.

• Gives written instructions and materials to patients and families.

• Reviews preventative health measures to patient and family. Documents understanding of instructions.

• Completes all charts/forms accurately and timely.

• Receives and gives a knowledgeable and concise patient report to others, as necessary to demonstrate awareness of patient’s overall condition.

• Initiates Clinical Pathways/Plan of Care on appropriate patient population at patients first visit.

• Updates and initiates Care Plans and Pathways as changes in patient’s needs are assessed.

Minimum Qualifications:

• Current licensure as a Registered Nurse in the State of Delaware required.

• ASN required, BSN preferred, from an accredited school of nursing

• At least 3 years' of clinical experience required, preferably in Wound Care.

• Wound Care certification preferred; will consider those with experience and able to obtain certification within 6 months of hire.
