Job summary

University City Rehabilitation and Healthcare Center is seeking a Wound Care Nurse to provide skilled wound assessment and treatment within a 124-bed sub-acute and long-term care setting.

The role requires an active PA LPN/RN license, BLS, and a minimum of 2 years in wound care within LTC. Wound care certification is preferred. Daily pay is available and the facility emphasizes compassionate, patient-centered care.

Qualifications

• Must possess a current, active LPN or RN license in PA.
• Wound Care Certification strongly preferred but not required.
• Valid BLS Certification (CPR) required.
• Minimum of 2 years of wound care nurse experience in long-term care.
• Strong clinical assessment and critical thinking skills.
• Ability to work effectively in a fast-paced healthcare environment.

Responsibilities

• Provide resident care per nursing standards, physician orders, and care plans.
• Assess, treat, and monitor wounds with sound clinical judgment.
• Deliver wound care treatments based on resident needs and best practices.
• Provide compassionate, person-centered care to residents, families, and caregivers.
• Offer clinical guidance and direction to nursing and care teams.
• Maintain accurate documentation of wound assessments, treatments, and outcomes.
• Maintain HIPAA-compliant confidentiality.
• Collaborate with interdisciplinary teams to support healing and prevention.

Responsibilities for Wound Care Nurse:

• Provide resident care in accordance with acceptable nursing standards, physician orders, and established care plans.
• Demonstrate sound clinical judgment in assessing, treating, and monitoring wounds.
• Provide wound care treatments based on resident needs and best clinical practices.
• Deliver respectful, compassionate, and person-centered care to residents, families, and caregivers.
• Provide clinical guidance and direction to members of the nursing and care team.
• Maintain accurate and timely documentation of wound assessments, treatments, and outcomes.
• Ensure resident confidentiality is maintained at all times in accordance with HIPAA and facility policies.
• Collaborate with interdisciplinary teams to support healing, prevention strategies, and continuity of care.

Qualifications for Wound Care Nurse:

• Must possess a current, unencumbered, active LPN or RN license in the state of PA.
• Wound Care Certification strongly preferred but not required.
• Valid BLS Certification (CPR) required.
• Minimum of 2 years of prior experience as a Wound Care Nurse in a nursing home or long-term care setting required.
• Strong clinical assessment and critical thinking skills.
• Ability to work effectively in a fast-paced healthcare environment.
