Job summary

Plymouth Hospital, affiliated with SoftPath Medical, is seeking a registered nurse licensed in Massachusetts to coordinate patient care from referral to discharge and follow-up. Epic EHR experience and MS Office proficiency are required.

Qualifications

Responsibilities

• Collaborate with physicians and healthcare team to coordinate patient care through discharge and follow-up.
• Assist with pre-authorization process.
• Serve as liaison for receiving consults and patient information.
• Coordinate scheduling of patient appointments, diagnostics, and procedures.
• Document patient evaluations and assist in ordering tests.
• Educate patients and families about care; develop educational materials.
• Participate in rounds and case conferences; pursue ongoing professional development.

Minimum Qualifications

• Registered Nurse license required in the Commonwealth of Massachusetts.
• Certification in wound, ostomy, and continence is not required.
• 1-3 years of related work experience preferred.
• Experience with computer systems is required, including web-based applications and some Microsoft Office programs such as Outlook, Word, Excel, PowerPoint, or Access.
• Experience with Epic electronic health record system is required.

• Bachelor’s degree in nursing preferred.

Essential Duties and Responsibilities

• Collaborate with physicians and other healthcare providers to coordinate patient care from referral and consultation through discharge and follow-up.
• Assist with the pre-authorization process.
• Serve as a contact and facilitator for receiving consults and patient information.
• Coordinate and ensure scheduling of all patient appointments, including consultations, diagnostic tests, and treatment procedures.
• Collect information for patient history and physical examinations.
• Perform and document patient evaluations and assessments.
• Assist in ordering laboratory tests, x-rays, and other diagnostic procedures.
• Work with surgeons and physicians to educate patients and their families about medical care.
• Contribute to the development of patient and family educational materials.
• Participate in patient rounds and case conferences.
• Maintain and update clinical knowledge and skills through consultation, formal and informal education, and literature review for self and clinical team.
• Provide leadership in process improvement related to patient care.
• Act as liaison and clinical expert to the Department of Quality & Safety for skin and wound-related serious reportable events.
• Lead quarterly pressure injury prevalence survey efforts for the hospital as part of NDNQI.
• Maintain annual mandatory education requirements.
• Demonstrate commitment to patients and colleagues, reflecting the goals of the hospital in nursing practice.
• Exhibit professional responsibility.
• Participate in educational opportunities for professional growth, personal well-being, and license renewal requirements.

Additional Requirement: Candidates must not have worked in permanent, travel, or PRN roles at any related facilities within the past six months.
