Qualifications

Responsibilities

• Collaborates with physicians and health care providers to coordinate patient care from referral to discharge/follow up.
• Assists with pre-authorization process.
• Serves as contact and facilitator to receive consults and patient information.
• Coordinates and ensures scheduling for consultations, diagnostics and treatments.
• Performs evaluations and assessments of patients and documents findings.
• Assists in ordering labs, x-rays and diagnostic procedures.
• Educates patients and families in medical care and participates in rounds and conferences.
• Leads process improvement related to care for wound patients and liaises with QA/Safety.
