Job summary

Kahak Health Care Services is seeking a registered nurse to provide in-home health care in the District of Columbia. The role involves assessment, planning, interventions, and evaluation of patient needs, including SOC/SOC recertification and SN visits.

You will maintain clinical documentation, educate patients and caregivers, and coordinate referrals. HIPAA compliance and participation in QA audits are essential components of this position.

Qualifications

• Must be a licensed RN with DC authorization and no exclusions.
• Active CPR & First Aid certifications.
• Ability to document and manage clinical information accurately.

Responsibilities

• OASIS Start of Care (SOC) and Recertification.
• Perform Skilled Nursing visits and 62 Day Summary Reports.
• Clinical documentation and other nursing duties assigned by KHCS.
• Maintain HIPAA-compliant patient confidentiality.
• Educate and counsel patients and caregivers on nursing needs.
• Initiate preventive and rehabilitative nursing procedures.
• Coordinate referrals to other services as needed.
• Participate in audits and quality assurance reviews.

We are recruiting a registered nurse for home healthcare services. We are looking for compassionate and capable professionals dedicated to the care of the individuals we serve.

Duties of the RN Position

You will be responsible for ensuring quality patient care through assessment, planning, interventions, and evaluation of patients needs as well as

• OASIS Start of Care (SOC) &OASIS Recertification
• Skilled Nursing (SN) Visits
• Sixty-Two (62) Day Summary Reports
  
• Clinical documentation and other nursing duties assigned by KHCS.
• Accurately and completely prepares all clinincal documentation and submits within the established timeframe per agancy policy and state regulations.
• Personal Care Aide (PCA) Supervisory Visits
• Taking and ensuring effectiveness measures
• Providing services and/or treatments requiring substantial and specialized nursing skill
• Educating and counseling patients and caregivers in meeting nursing and related needs.
• Initiating appropriate preventative and rehabilitative nursing procedures and provides referrals to other services and disciplines as needed.
  
• Maintain patient confidentiality in accordance with HIPAA.
• Participating in personal and professional growth and development and maintains current knowledge of industry guidelines, clinical skills and competency
  
• Cooperate with audits and Quality Assurance reviews.

Compensation is based on a fee schedule for services.
