Job summary

KAHAK Health Care Services (KHCS) is seeking a qualified Registered Nurse to deliver patient-centered, evidence-based care in a home health setting in Washington, DC. You will work in a collaborative, inter-professional team focused on safety, quality, and compassionate care.

The RN will assess, educate, and advocate for patients, coordinate with care staff, and ensure documentation meets agency standards while supporting growth and professional development.

Qualifications

• DC licensedRN required versus general RN licensure?
• BLS or ACLS certification required or preferred.
• Experience with Home Health patients is preferred.

Responsibilities

• 1) Performs initial and ongoing comprehensive assessments of the client's needs.
• 2) Administers treatments per physician orders and in accordance with nursing standards of practice.
• 3) Initiates the plan of care and assigns tasks to LPNs and HHAs as needed.
• 4) Provides services requiring specialized nursing skills.
• 5) Educates and counsels patients and caregivers in treatment needs.
• 6) Refers to other services as needed and ensures coordination of care.
• 7) Maintains accurate clinical documentation within policy timelines.
• 8) Communicates changes in patient condition to physician and updates orders as needed.
• 9) Collaborates with family and home care team to meet patient goals cost-effectively.
• 10) Participates in performance improvement activities and meetings.
• 11) Supervises LPNs and paraprofessionals.
• 12) Attends staff meetings and training; supports professional development.
• 13) Maintains knowledge of industry guidelines and competency.
• 14) Fulfills additional duties as assigned.

Job Description Summary:

The RN provides patient-centered evidence-based nursing care through an inter-professional approach to treatment, education, and advocacy. Job Description:

• 1. Performs initial and ongoing comprehensive assessments of the c1ients needs. including assessments or re-evaluation at specified time points;
• 2. Administer treatments based on patient’s identified needs, physician’s order and in accordance with nursing standards of practice.
• 3. Initiates the plan of care, makes specific patient assignments to LPNs and HHAs as necessary
• 4. Provides services and/or treatments requiring substantial and specialized nursing skill;
• 5. Educates and counsels the patient and caregivers in meeting their treatment and related needs;
• 6. Initiates appropriate preventive and rehabilitative nursing procedures, and provides referrals to other services and disciplines as needed;
• 7. Accurately and completely prepares all clinical documentation and submit within the established timeframe per agency policy;
• 8. Identifies and communicates changes in the patient's condition; informs the physician and obtains new orders as needed;
• 9. Maintains communication with the patient family and other members of the home care team to ensure coordination of services and episode management that meets patient goals in a cost-effective manner;
• 10. Evaluates outcomes of care through performance improvement activities, clinical record review. and other assigned duties;
• 11. Supervises LPN/s and paraprofessionals providing home care services;
• 12. Participates in patient care conferences. staff meetings and in-service programs at least quarterly;
• 13. Participates in personal and professional growth and development;
• 14. Maintains current knowledge of industry guidelines. clinical skills and competency to be a resource to staff;
• 15. Fulfills additional performance responsibilities as assigned and as necessary.

QUALIFICATIONS:

Must be a D.C Licensed Registered Nurse

Hold one of the following License or Certification; BLS, ACLS

Experience working with Home Health patients is preferred
