Qualifications

• Current RN license for Washington, DC, without exclusions from participation in any federal or state healthcare program.
• Valid driver’s license and reliable vehicle.
• Current CPR & First Aid certification.
• US work authorization (US Passport, US Work Authorization card, Permanent Resident Card) or US birth evidence.

Responsibilities

• Assess, plan, implement, and evaluate patient needs in home healthcare.
• Perform OASIS Start of Care (SOC) and Recertification, SN visits, and 62-day summary reports.
• Maintain accurate clinical documentation and HIPAA compliance.
• Provide patient and caregiver education and counseling on nursing needs.
• Coordinate with KHCS team and other disciplines; participate in QA and audits.
