Transitions Hospice is seeking a Registered Nurse Visit RN for PRN ECT coverage in Champaign, IL. You will provide flexible, skilled nursing support across the care team, ensuring timely patient assessments and interventions in home settings.

The role requires an active RN license, nursing degree, hospice/home health experience preferred, and ability to travel locally. Competitive pay and company-provided equipment are offered.

Responsabilidades

• Deliver professional nursing care across the full nursing process within your scope of practice.
• Conduct thorough patient and family assessments and contribute to individualized care plans.
• Complete all visit types as needed to meet patient and family needs.
• Administer medications and treatments per physician orders.
• Identify nursing diagnoses from health assessment data and respond accordingly.
• Record accurate, timely clinical notes including pain and symptom management outcomes.
• Communicate changes in patient condition to physicians and obtain orders as required.
• Coordinate with community health partners to support seamless care transitions.
• Ensure equipment and services are arranged and available for patients.
• Maintain and grow clinical skills through continuing education and in-service participation.

As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:
