Transitions Hospice is seeking an experienced Registered Nurse to join our Extended Care Team as a PRN visit RN in Aurora, IL. You will provide skilled nursing care, assessment, and coordination for patients in the Northern Illinois territory, delivering care across visits as needed.

You must hold an active RN license in Illinois, graduate from an accredited nursing program, and be comfortable traveling locally.

• Experience in hospice, home health, or related setting preferred
• Flexibility and confidence across varied patient assignments
• Strong assessment skills and clinical judgment
• Excellent documentation habits and communication skills
• Valid driver’s license and local travel willingness

Responsibilities

• Deliver professional nursing care within the full nursing process
• Conduct patient and family assessments and contribute to care plans
• Complete all visit types as needed to meet patient/family needs
• Administer medications and treatments per orders
• Identify nursing diagnoses from assessments and respond accordingly
• Record timely clinical notes including pain/symptom outcomes
• Communicate changes in patient condition to physicians and obtain orders as required
• Coordinate with community health partners for seamless care transitions
• Ensure equipment and services are arranged for patients
• Maintain and grow clinical skills through continuing education

As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:
