Job summary

Valor Healthcare is seeking a Registered Nurse to assist practitioners with patient care, physical examinations and ancillary tests, collaborating with the PACT Team and VA providers to develop patient care plans.

You will educate patients and families, triage unscheduled visits, maintain confidentiality, and ensure compliance with VA and agency training requirements, while delivering excellent service to veterans in a multi-disciplinary environment.

Qualifications

• RN with Associate degree required.
• 2-3 years' experience in primary or ambulatory care, government healthcare preferred.
• Active license in the US; VA contract requirements apply.
• BLS and ACLS certifications as specified by VA contract.
• Strong English communication and EMR proficiency.
• Safety-sensitive position.

Responsibilities

• Actively assists practitioners with patient care, physical examinations and tests.
• Triages unscheduled patients and supports PACT Team leadership.
• Maintains confidentiality of veterans' data and information.
• Provides patient and family health education focusing on self-management and wellness.
• Participates in clinic outreach and enrollment activities.
