Job summary

Valor Healthcare in Sherman, TX is seeking a Registered Nurse to support practitioners with patient care, examinations, and tests within a PACT team framework. This on-site role emphasizes coordination with VA and community partners to deliver high-quality, patient-centered care to veterans.

You will triage patients, educate families, maintain confidentiality, and comply with VA and company training requirements, while advancing self-management and wellness across care plans.

Qualifications

• RN with Associate Degree required.
• 2–3 years’ experience in primary/ambulatory care, government healthcare environment preferred.
• Must be credentialed and in good standing with VA.
• State license in good standing for desired employment region.
• BLS and ACLS certifications as specified by VA contract.

Responsibilities

• Assist practitioners with patient care, physical examinations and ancillary tests.
• Triages unscheduled patients requesting care.
• Act as point person within PACT Team.
• Provide patient and family health education focused on self-management, prevention and wellness.
• Maintain confidentiality of veterans’ data and information and comply with training requirements.
• Coordinate with VA and non-VA providers and facilities as needed.
• Support patient enrollment and retention in appointments and nurse visits.
