Job summary

Jobtailor is seeking a Registered Nurse to join our care coordination team in Colorado. The role focuses on patient calls, med reviews, education, and walk-in care, coordinating with physicians and staff to ensure continuity of care.

You will utilize EHR systems and collaborate across departments to optimize patient outcomes. The ideal candidate holds an RN license with BLS, has five years of clinical experience, and is capable of autonomous decision-making with strong clinical judgment.

Qualifications

  • Bachelor's degree in nursing or related field.
  • Current RN license in state of practice, or compact licensure.
  • Current BLS certification.
  • Five years of clinical experience.
  • Excellent organizational skills and clinical knowledge in specialty care.
  • Ability to make autonomous decisions with strong clinical judgment.
  • Bilingual ability is a plus and willingness to float to Loveland/Greeley as needed.

Responsibilities

  • Manage and prioritize inbound patient calls and electronic messages requiring assessment and intervention.
  • Conduct medication reviews.
  • Provide patient education and follow-up care coordination.
  • Provide direct patient care for walk-in patients.
  • Collaborate with cardiologists, medical assistants and staff to support continuity of care.
  • Facilitate referrals, diagnostic testing and treatment follow-up as directed by providers.
  • Evaluate calls using protocols, databases and clinical judgment.
  • Assess acuity and direct callers to appropriate care or resolution.
  • Research patient history using EHRs and databases.
  • Document calls, assessments, interventions and disposition plans.
  • Participate in quality assurance and improvement processes.
  • Consider patient needs including physical, cultural, psychosocial and age-specific factors.

Job description

  • Evaluate member/patient calls and needs using established protocols, databases, best-practice evidence and clinical judgment
  • Assess acuity and direct callers to appropriate care or resolution
  • Research patient history using multiple databases and electronic health systems/EHRs
  • Access symptom-based guidelines and document calls, assessments, interventions, responses and disposition plans
  • Participate in quality assurance and improvement processes
  • Consider physical, cultural, psychosocial, spiritual and age-specific needs when planning care
  • Connect patients with appropriate services and coordinate care
  • Provide direction and supervision to licensed and non-licensed personnel
  • Interact with patients, families, physicians, healthcare providers, departments and external community contacts
Requirements
  • Bachelor's degree in nursing or related field
  • Current, valid RN license in state of practice, temporary RN license in state of practice, or compact RN licensure for current state of practice
  • Current BLS certification required for state of practice
  • Additional certification or continuing education may be required based on area of practice
  • Proficiency level typically achieved with five years of clinical experience
  • Excellent organizational skills and clinical knowledge regarding specialty care services
  • Knowledge of care coordination of services, legal and financial aspects of diagnostic services and health services in specialty area
  • Ability to make autonomous decisions utilizing excellent clinical judgment
  • Highly effective interpersonal and communication skills
  • Understanding of quality customer service principles
  • Effective communication and writing skills
  • Good time management skills
  • Knowledge of word processing and database software applications
  • Ability to teach clinical and non-clinical personnel regarding care and diagnostic services
  • Good understanding of process improvement
  • Bilingual ability a plus
  • Must be willing to float to Loveland and/or Greeley for staffing support if needed

Demonstrates expertise in patient care coordination, medication reviews, and clinical judgment while effectively collaborating with healthcare teams. Proficient in utilizing electronic health records and databases to assess patient needs and facilitate quality improvement processes.
