Jobtailor is seeking a dedicated healthcare coordinator to assist patients through the care journey. You will guide patients, explain diagnoses and treatment options, and coordinate with providers, social workers, and community services to ensure timely access to care.

Qualifications

Responsibilities

  • Guide patients through the healthcare system and help them manage their care journey.
  • Assist patients in understanding diagnoses, treatment options, and the healthcare process.
  • Serve as a communication link between patients and care providers.
  • Collaborate with healthcare providers, social workers, and other team members.
  • Facilitate appointments for consultations and support services.
  • Help patients overcome barriers to care and connect with resources.
  • Collect data and prepare reports for quality initiatives and programs.
  • Support clinical work and procedures within policy and procedure.

Job description

  • Assist patients in understanding diagnoses, treatment options, and the healthcare process
  • Serve as a communication and assessment link between patients and care providers
  • Collaborate with healthcare providers, social workers, and other team members
  • Facilitate appointments for consultations and support services
  • Help patients overcome logistical and other barriers to care
  • Connect patients with community resources, support groups, counseling services, and local organizations
  • Collect data and prepare reports for quality initiatives and programs
  • Perform clinical work and/or assist with clinical procedures within the specialty area according to policy and procedure
Requirements
  • Licensed or certified in area of specialty, or have a Bachelor’s degree and 3 years of healthcare experience in lieu of certification (preferred)
  • Certification and registration with ARRT, RDMS, or CNMT, plus Medical Imaging State Licensure (preferred unless required by the state)
  • Licensed as a Social Worker in state of practice
  • Licensed Practical Nurse (LPN) in state of practice
  • Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), Certified Clinical Medical Assistant (CCMA), NRCMA, or NCMA certification
  • BLS Basic Life Support – American Heart Association; preferred upon hire, required at hire for RSFH
  • High school diploma or GED (required)
  • 3 years of healthcare-related experience working with patients in care coordination or direct patient care (required)
  • Ability to provide care appropriate to patients’ ages and developmental needs
  • Ability to exercise universal precautions and use personal protective equipment
  • Verbal and written communication
  • Attention to detail and critical thinking
  • Patient observation, discharge planning, mail management, patient advocacy, organization, task management, and time management
